Background Statement

Encompass Housing was established in 2023 and is a 501(c)(3) nonprofit organization with a mission to build communities that foster a sense of belonging, while creating safety nets that provide support and stability. We are dedicated to serving seniors and families with children and we strive to provide not just housing, but a foundation upon which individuals can build their lives.

Encompass Housing provides housing supports that aim to eliminate barriers to safe and permanent housing. Housing navigators assist our clients throughout the entire process of securing and maintaining safe and stable housing. We provide assistance with maintaining stable tenancy once housing is secured and also assist with housing security deposits, utility setup fees, first and last month’s rent, and the first month of utilities.

On March 19, 2024, our Recuperative Care Center and Short-Term Post-Hospitalization services at Community Care Campus Long Beach (CCCLB) opened to the public, expanding our ability to address housing inequity and improve care coordination for those experiencing homelessness. We offer 72 Recuperative Care beds and 30 Short-Term Post Hospitalization beds. These services offer support to individuals regaining their strength, functionality, and independence after a hospital stay.

NHHA's Community Care Campus is designed to provide an integrated approach to caring for the whole person. The campus connects the local community with services that address their needs—ranging from physical and mental health to supportive housing. We serve as a safety net for the most vulnerable community members, who, without our care, support, and intervention, might not otherwise have access to essential healthcare services. Our comprehensive care model integrates healthcare delivery with effective interventions that respond to our patients' social determinants of health (SDOH), ensuring that those who would otherwise be unable to receive care are not left behind.

Impact Statement

Over the past year, our organization has achieved significant milestones, including providing Housing Navigation Services to over 400 individuals and families, housing 100 individuals since January 2024, and serving more than 120 individuals in our Recuperative Care program. Looking ahead, our goals for the upcoming year include expanding our Housing Supports program to Long Beach, Los Angeles, and the Inland Empire, housing 150 individuals by December 31, 2024, and opening the Hatfield facility to increase our capacity to serve clients in need of Recuperative Care.

Needs Statement

1) Funding and Resources: Securing consistent financial support is essential to meet the needs of our growing organization. We continue to seek diversified funding.

2) Sustainable Housing Solutions: A significant need is the availability of affordable and permanent supportive housing options. The shortage of housing presents a challenge in effectively housing homeless individuals.

3) Access to Healthcare Services: Establishing partnerships with healthcare providers is vital for comprehensive healthcare, including mental health and substance abuse treatment.

4) Case Management and Support Services: Effective case management is crucial for addressing the complex needs of the homeless population. Finding and retaining skilled case managers for this vulnerable group is imperative.

5) Community Outreach and Engagement: Building relationships with the homeless population is key to increasing trust. Strengthening outreach programs continues to be a need
