Housing Services, Recuperative Care, Enhanced Care Management

We assist individuals or families that are homeless or at risk of homelessness in locating and securing housing, covering housing deposits, and providing support to ensure that housing is retained. The Housing Services we offer aim to eliminate barriers to safe and permanent housing solutions. These services include Housing Transition Navigation Services, Housing Tenancy & Sustaining Services, and Housing Deposits. Encompass Housing is also proud to serve the local community with Recuperative Care services. Recuperative Care is a program for individuals who are exiting the hospital and need a supportive and safe environment to rest and recover. Recuperative Care bridges the gap between hospital and home, offering medical supervision and support in a stable environment by allowing a safe, clean environment for individuals to continue their healing process after leaving the hospital/emergency room, providing a bridge for those experiencing homelessness to transition from hospitals/acute care to housing, reducing costs for the system as well as unnecessary emergency room visits and 911 calls, and creating a support network that can provide individuals with enhanced care coordination and connection to resources. Enhanced Care Management (ECM) is a service model designed to provide comprehensive, coordinated, and client-centered care for individuals with complex health and social needs. This approach aims to improve health outcomes by addressing both medical factors and the social determinants of health that contribute to a person's overall well-being. Individuals will receive support with medical care coordination, finding doctors, scheduling appointments, managing medications, and setting up transportation, applying for other community-based services, communication and coordination with entire care team, and health education and advocacy.
