Street Outreach

Our case managers go out in teams, often with a Homeless Liaison Office from the local police department, to find and connect with local neighbors experiencing homelessness to build trust, build connection, and hopefully find housing options for them so they can get off of the street. These connections can also be made via "inreaches", which are regular opportunities at local churches, agencies, or events where people can come to a central locale to register for services. Sometimes a neighbor experiencing homelessness can call our outreach phone number or our online portal and schedule an appointment with a case manager as well. Street outreach is service delivery for the specific purpose of reaching out to unsheltered homeless neighbors; connecting them with emergency shelter, housing, or critical services; and providing urgent, non-facility-based care. These activities are intended to help homeless neighbors to begin the process of obtaining appropriate supportive services, including assistance in obtaining permanent housing, medical health treatment, mental health treatment, counseling, supervision, and other services essential for achieving independent living. City Net’s street outreach services include engagement services, emergency health/mental health, transportation and services for special populations.

Outcomes

Ultimately, it is up to our clients, their health, and the availability of beds in a shelter or permanent supportive housing that determines this. We have seen clients get off the street in a day, and some have taken the better part of a year once we take their case. It is our desire to see people experiencing street-level homelessness to exit that condition via the following ways:
• Reunification with their family
• Admittance into a rehab program lasting 60 or more days
• Admittance into a long term shelter
• Finding permanent supportive housing

We do not count it as a street exit for our numbers if a person:
• Is admitted into a short term or emergency shelter
• Is admitted into a rehab program 30 days or less
• Is incarcerated
• Is hospitalized
• Is deceased

Housing Navigation

Housing navigation connects a homeless individual with a trained and experienced housing navigator to develop a housing plan, address the barriers identified during the plan, and acquire documentation and complete forms required for housing. It also involves housing search which includes attending property owner meetings, setting appointments, and assisting with paperwork related to housing applications. Navigation also includes tasks involved with securing housing including inspections, utility startups, and moving into housing.

Outcomes

The goal of all housing navigation is to provide permanent supportive housing to our neighbors experiencing homelessness, that they can then use this as a base to grow into more holistic health and wellbeing.

Case Management

Case management activities assess housing and service needs and arrange, coordinate and/or monitor the delivery of individualized services. Specific activities include: • Assessment and intake using HMIS (Homeless Management Information System); • Conducting initial evaluations including, verifying and documenting eligibility; • Counseling; • Developing/securing/coordinating services; • Helping obtain Federal, State, and local benefits; • Monitoring and evaluating program participant progress in particular programs; • Providing information and referrals to other providers; and • Developing an individualized housing stabilization plan that leads to the attainment of stable permanent housing.

• including them in the HMIS database for more service options
• obtaining ID's
• permanent supportive housing
• benefits
• counseling
• a continuum of care

Census Coordination

We assist and/or lead in Point In Time Counts or Homeless Censuses as we are contracted by different cities or regions.

Outcomes

We desire to give all regions that contract a census from us to be able to give them an accurate and timely count of all the people in their borders who are experiencing homelessness. This helps greatly in assessing the needs, requirements, and eligibility for services, funding, and housing.

Non-Congregate Shelter Management

We operate several temporary and indefinite non-congregate shelter programs, including those that are low-barrier entry and transitional/temporary housing solutions. We believe in the dignity, safety, and privacy that non-congregate shelters provide, and work with many other shelter providers like Pallet, LifeArk, Boss Cubez, etc.

Outcomes

Our shelter programs provide transitional housing for our clients. It is our desire to see our shelter guests find permanent supportive housing and independent living. Each client has a case manager they work with to obtain housing, all while receiving meals, job training, life skills classes, and much-needed community.
