Background Statement

In 1977, Providence TrinityCare was established as one of the first hospice programs in the nation, starting as a small, local service that developed into a regional program with multiple interdisciplinary teams and a dedicated staff serving all of Los Angeles and Orange Counties. We provide a full range of palliative care, hospice care, and compassionate support to thousands of patients and their families. In 2001 we launched the only dedicated pediatric hospice and palliative care program serving all of Los Angeles and Orange counties.
Recent accomplishments of TrinityCare include:
Became Joint Commission Certified (JAHC0) in 2020.
Expanded services across the continuum covering all hospice and palliative care programs throughout Providence (California, Oregon, Washington and Texas).
Providence TrinityCare Hospice and Palliative Care received the highest caregiver experience scores.
Providence TrinityCare’s Chief Medical Officer Dr. Glen Komatsu, was the runner up for the Schwartz Center National Compassionate Caregiver of the Year Award
Through philanthropy TrinityCare secured funding to design and create a Japanese Meditation Garden to support our caregiver’s wellbeing and resilience.
Bereavement Care continues with all families up to 13 months after the death of a loved one. Support groups there are available for ages five – adult including siblings, parents, surviving spouses and any other family member needing help processing a loss.

Needs Statement

The Providence TrinityCare Hospice Palliative Program is an innovative team approach to how patients with life-threatening illness are supported and cared for during any stage of their illness. With physician leadership to oversee and deliver outpatient services, a Registered Nurse to deliver direct patient care, a Licensed Clinical Social Worker to follow patients physically unable to travel to outpatient sites, and an administrative leader to ensure operational success. The program team will make a combination of home visits, office appointments, and phone calls for assessment, intervention and care coordination.

Statement from the CEO/Executive Director

It is only because of the community and people like you that we are able to fulfill our mission to serve all patients and families in need, including the poor and vulnerable.
Our TrinityKids Care Pediatric Hospice program continues to serve more patients and families, and because of our foundation’s support, all regional children’s hospitals have local liaisons to help support families with their transition of a child to hospice services.
Community Palliative Care continues to grow and serve more patients through long term illness or chronic disease management.
Necessities of Life provides families in need with basics such as food, clothing, household items and utility bill payments. Many have no where to turn when someone is passing and job loss or extended time away from earning income puts many families in impossible situations that we help navigate.
Patient Care Assistance supports all who have no insurance or financial resources for care as no one is ever turned away for ability to pay.
Caregiver Education & Fellowships help us train the workforce that delivers our whole person care to our communities.
Endowments secure leadership, vision and new ideas that propel our ability to attract quality caregivers with continuing education, training, and support.
Bereavement Services are offered to all families upon the death of a loved one and include more than a year of support by our social workers to all in need. Groups are also open to the larger community and programs are almost entirely funded by philanthropy.
Caring for our sick and dying, our poor and vulnerable and fulfilling the mission of Providence is only possible because of the generosity of people like you, thank you.

Statement from the Board Chair/President

Working for Providence Hospice & Palliative Care is more than a job—it’s a calling. Ask any physician, nurse, social worker, chaplain, or caregiver. They will proudly tell you what it means to practice whole-person care. The Sisters of Little Company of Mary built the foundation for our services in 1986, and that legacy has guided our growth since. Our teams continue in that spirit today by caring for and easing the way for all they serve.
Our caregiving staff is the eyes and ears for the well-being of our families and the conduit for our Necessities of Life program. Disparities magnified by the pandemic have prompted an increase in requests for assistance. Families living with the dual burden of a terminal illness and COVID restrictions have been impacted severely. Examples are endless, including families becoming homeless due to layoffs, lack of urgent basic needs like diapers and food, and the inability to afford critical medications and medical equipment.

Our staff impacts lives of our community seven days a week, 365 days a year. As they deliver wraparound care to pediatric patients and adults in every department, these caregivers make a profound difference. We are coming out of an incredibly challenging two years where their resolve was constantly tested. As they found creative, compassionate ways to hold our families close, they turned to each other for much-needed support.
