LIFELONG LEARNING for TRANSITION AGE YOUTH (Ages 18+)
Our instructors and clinical staff: • Adapt projects and experiences to personal learning styles and abilities • Provide clinical support including Occupational, Physical and Speech Therapies • Assist with transportation navigation • Provide training from money management and budgeting to nutrition and meal-planning. Digital Literacy and Equity: • Internet access, usage and safety • Basic computer skills • Resume development • Job search and preparation • Online connection with family and social network
Participants can expand functional and life skills while identifying personal areas of interest, optimizing growth and creating a path to greater independence using the tools and abilities necessary to live successfully as adults.
INDEPENDENT LIVING SERVICES
Staff provide one-to-one training to individuals who can live successfully in the community, with support. Services include: Health, fitness and medical management Budgeting, meal preparation and housekeeping Recreation and community access Teamwork training and workplace adaptation Socialization and healthy communication Benefits management, banking, money management
Participants receiving independent living support are better able to understand and manage the tasks of daily living, minimizing risk of homelessness or unwanted family care, and maximizing their ability to live as contributing members of our community.
LIFELONG LEARNING for ADULTS (Community Based Adult Services)
Adults of all abilities are supported and empowered through campus and community-based learning that engages and enriches participants through classes, activities and local partnerships that foster continued growth and greater independence. The program stresses partnership with the participant, the family, the physician, and the community in working towards maintaining one's optimal capacity. A robust multidisciplinary team that includes an occupational, physical, speech therapist, dietician, pharmacist, nurse, and social worker develop an individualized care plan that focuses on health and wellness services that the participant receives daily while at the program. Our instructors and clinical staff: • Adapt projects and experiences to personal learning styles and abilities • Stress partnerships with each person, their family, physician and their community • Provide clinical support including Occupational, Physical and Speech Therapies • Support adults in accessing higher education at local community colleges • Assist with transportation navigation • Include subjects from general academics, life skills, art, and music Functional Academics and Life-Skills: • Includes subjects from general academics (reading, math, science) plus health and fitness classes, art and music • Includes life-skills training from money management and budgeting to nutrition and meal-planning Digital Literacy and Equity: • Internet access, usage and safety • Basic computer skills • Workforce orientation and job exploration • Job search and preparation; resume building • Online connection with family and social network • Telemedicine
The success of our program for adults can help to keep medically fragile and participants with disabilities from needing acute care or institutionalization. Instead, these program participants will continue to build on their abilities, realize their highest potential and live with dignity and purpose in the community.
CHILD DEVELOPMENT PROGRAM
Reimagine's Child Development Program provides center based early intervention services in a group setting for children ages 18 months - 36 months who are at risk or show evidence of developmental delays. Children who are born prematurely, prenatally exposed to drugs, or with specific conditions, diagnoses or special needs may qualify for services. A multi-disciplinary team of professionals is assigned to work with the child and family. Staff and clinicians focus on specific goals and objectives in areas that may include: Gross and Fine Motor Skills, Speech, Language and Cognition, Sensory Development, Self-Help, Social/Emotional Skills, among other areas. Young participants receive services and support including: - Cognitive development through play - Sensory-based therapies - Specialized equipment and motor skills therapy - Physical, occupational and speech therapies - Social development activities
Children can continue on a path to reach developmental milestones with improvements in speech and communication, mobility, spatial awareness, social engagement and emotional stability. With support, these children can enter pre-school alongside their peers.
