Bereavement Support

The loss of a loved one can be overwhelming and isolating. The Southern California Hospice Foundation offers free virtual and in-person bereavement workshops designed to promote healing, self-care, and emotional resilience. Sessions are led by certified bereavement counselors and provide participants with practical tools, peer support, and guidance during the grieving process.

Heavenly Home

Opened in February 2023, the Heavenly Home is Orange County’s first and only nonprofit residential end-of-life care home. The program provides 24-hour caregiving in a comfortable, home-like environment for individuals facing the final stage of life who are unable to remain safely in their own homes. The Heavenly Home offers physical, emotional, and spiritual support, allowing families to focus on connection, reflection, and meaningful moments together.

Residents will receive compassionate, continuous care in a safe and dignified environment.

Families will report reduced caregiving burden and increased ability to spend meaningful time with their loved ones during end-of-life.

Angel Assistance Program

The Angel Assistance Program addresses critical needs that are not covered by Medicare, Medi-Cal, or private insurance. Support ranges from transportation and basic necessities to complex, deeply meaningful final wishes. Whether providing immediate relief or creating a lasting memory, each request reflects SCHF’s commitment to dignity, compassion, and individualized care.

Patients and/or families will complete a follow-up evaluation within one month indicating satisfaction with the assistance provided.

Families will report reduced stress and increased emotional well-being as a result of program support.

Community Outreach Program

Many families report wishing they had known about hospice care earlier. SCHF’s Community Outreach Program works to increase awareness and understanding of hospice and end-of-life resources throughout Southern California. Through educational materials, community presentations, healthcare partnerships, and media engagement, SCHF encourages earlier access to hospice care and promotes informed decision-making around serious illness and end-of-life planning.

We Honor Vets

The We Honor Vets program recognizes and supports terminally ill Veterans by addressing the unique physical, emotional, and psychological challenges they may face at the end of life. The program provides education to care teams and facilitates pinning ceremonies that honor Veterans’ service through formal recognition, certificates, and lapel pins—affirming dignity, gratitude, and respect.

Conduct 50 Veteran pinning ceremonies annually, led by SCHF’s Military Advisor and supported by Junior Reserve Officers’ Training Corps participants.

Veterans and families will report increased feelings of recognition, closure, and emotional support.
