Integrated Housing and Healthcare Services
Family Emergency Homes Our Family Emergency Homes are homes that are ready to welcome families who need a safe place to stay as we work with them to address their specific needs on their journey to self-sufficiency. Illumination’s Children and Families Program provides trauma-informed mental health and family stabilization services designed to help children and parents heal from trauma, with the goal of disrupting the intergenerational cycle of homelessness. Programming includes intensive case management, parenting courses, children’s developmental screening, mental health services, academic enrichment, and housing navigation. We partner with CHOC to provide same-day pediatric visits and specialty care. Services currently take place at five Illumination family emergency homes. Shelters/Navigation Centers We operate two navigation centers in Orange County where clients can stay 24 hours a day and receive integrated housing and healthcare services. Clients receive an intensive level of services to increase income, connect to resources, secure housing, and cope with underlying medica, behavioral health and substance use issues that present barriers to housing. Every service is designed to promote forward movement into housing rather than cyclical movement in and out of shelters. Recuperative Care/Medical Respite We operate seven recuperative care facilities throughout four counties, totaling more than 500 beds. Recuperative care provides safe bridge housing where people experiencing homelessness can recuperate after emergency room or hospital discharge. Our program greatly reduces the cost to the community by taking homeless individuals from hospital beds into our care. Clients are provided medical oversight, intensive case management, behavioral health therapy, and substance use counseling. Children and Families Recuperative Care Illumination Health + Home is opening a Children and Families Recuperative Care in Santa Ana. This is the nation's first recuperative care facility for children and families experiencing homelessness. Illumination's Children and Families Recuperative Care Program will provide transitional housing to medically vulnerable homeless children and their families. The program will offer a safe haven for children to recover from illness or injury post-hospitalization while addressing the complex challenges faced by their families. By providing comprehensive case management, housing services, and trauma-informed supportive services on-site, Illumination will create a nurturing environment for families to heal and stabilize. Richard Lehn Intergenerational Campus Illumination Health + Home is opening an intergenerational campus in Santa Ana to serve Transitional Age Youth families and seniors. This campus will address the unique challenges of families and seniors by integrating programming to foster a sense of community and mutual support. The campus also has an on-site preschool operated by Head Start, serving children from the campus and the surrounding neighborhood.
In 2023, we served nearly 7,000 clients. We housed 959 clients (714 adults and 245 children). We served 327 people (156 adults and 171 children) from 97 families in our family emergency homes. We provided 1,638 clients with recuperative care/medical respite.
