Integrated Housing and Healthcare Services
FAMILY EMERGENCY HOMES // Illumination Health + Home’s Family Emergency Homes welcome families who need a safe place to stay as we work with them to address their specific needs on their journey to self-sufficiency. Our Children and Families Program provides trauma-informed mental health and family stabilization services designed to help children and parents heal from trauma, with the goal of disrupting the intergenerational cycle of homelessness. Programming includes intensive case management, housing navigation, parenting classes, developmental screening for children, behavioral health services, and academic enrichment services. SHELTERS/NAVIGATION CENTERS // Illumination Health + Home operates two navigation centers in Orange County where clients receive an intensive level of services to increase income, connect to resources, secure housing, and cope with underlying medical, behavioral health, and substance use issues that present barriers to housing. Every service is designed to promote forward movement into housing rather than cyclical movement in and out of shelters. RECUPERATIVE CARE/MEDICAL RESPITE // Illumination Health + Home operates six recuperative care facilities throughout our service region, totaling more than 400 beds. By offering short-term medical oversight, case management, and care coordination, our recuperative care program helps patients recover fully and connect to ongoing services. This reduces preventable emergency department visits and hospital readmissions—ultimately lowering healthcare costs for hospitals, public systems, and the broader community. PERMANENT SUPPORTIVE HOUSING // Illumination Health + Home has developed and operated permanent supportive housing programs since 2014. Development projects have included: 1. ADU in Fullerton serving as permanent supportive housing for a family experiencing homelessness. 2. LifeArk El Monte, an 18-unit, permanent supportive housing community in El Monte. Additionally, we provide supportive housing services in Orange County at 17 scattered-site apartment units and 19 micro-communities. CHILDREN AND FAMILIES RECUPERATIVE CARE // Illumination’s Children and Families Recuperative Care Center, located in Santa Ana, is the nation's first recuperative care facility for children and families experiencing homelessness. The program offers a safe haven for children to recover from illness or injury post-hospitalization while addressing the complex challenges faced by their families. By providing comprehensive case management, housing services, and trauma-informed supportive services on-site, we have created a nurturing environment for families to heal and stabilize. RICHARD LEHN INTERGENERATIONAL CAMPUS // Illumination Health + Home is developing an intergenerational campus in Santa Ana that will address the unique challenges of Transitional Age Youth families and seniors by integrating programming designed to foster a sense of community and mutual support. The campus also has an on-site preschool that will serve children from the campus and the surrounding neighborhood.
In 2025, we served 6,049 individuals and helped 1,271 individuals secure housing. We served 73 families (105 adults and 160 children) in our family emergency homes. We provided 2,323 clients with recuperative care services.
