Permanent housing to support the mental and physical health of people living with Schizophrenia Spectrum Disorders, the hardest serious mental illness to treat successfully

Community-Based Supportive Housing Program that provides: - A Supportive Home on our open, smoke-free campus - Therapeutic Services based on evidence-based techniques that promote mental health stability -Health & Wellness Program for medication management and symptom oversight - Nutrition Program for healthy living - Peer Engagement & Life Skills for enhancing social skills and social interactions - Supported Work, Education and Volunteer opportunities - Family Involvement, which is integral to the continuum of care and the long-term well-being of our residents and their family members

Outcomes

- Ensure all community members remain medication compliant and involved in medically necessary treatment
- Keep hospitalization rates below 5%
- Maintain zero incidents of incarceration
- Facilitate active engagement in daily therapeutic and community activities
- Ensure all members receive annual physical, vision, and dental exams; manage specialized care appointments; provide advocacy for appointments as well as follow up
- Support stabilized members in returning to work, school, volunteering, using public transportation, and/or securing part-time employment
