Comprehensive Care

The program provides access to non-emergency direct medical care for patients in Orange County each year positively impacting health access and health outcomes. Comprehensive Care program activities include: • Walk-In Urgent Care: LBCC's urgent care is available six days per week with no appointment. Patients receive low-cost care for acute diagnoses ranging from simple colds and infections to potentially life-threatening conditions such as acute asthma, uncontrolled diabetes, and chest pain. • Primary and Preventive Care: Patients can establish a permanent Medical Home to regain control of their health. Patients of all ages can access preventive and primary care, including vaccinations, age-appropriate screenings, and weight and blood pressure monitoring. The Clinic’s full-time dietician sees patients with diagnoses that will benefit from nutritional guidance, as well as patients simply seeking a healthier lifestyle. • Specialized Care: LBCC offers long-term care for HIV/AIDS, diabetes, and hypertension, among other chronic conditions. The case management team ensures seamless access to contracted on- and off-site services such as radiology, after-hours nurse line, dental, and pharmaceutical. • Mental Health: On-site therapy and counseling address common issues including depression, anxiety, addiction, and eating disorders. Associated medical visits monitor physical health (medication side effects, sleep, nutrition) related to mental health diagnoses and to screen for mental health issues. • Insurance Counseling and Supportive Services: Two bi-lingual Covered California Certified Enrollment Counselors assist uninsured patients or newly unemployed patients to determine eligibility for Medi-Cal and assist in the application and enrollment process, to guide patients to fully access their Medi-Cal coverage. Transportation vouchers and translation services are available; case managers coordinate supportive services such as food pantries or WIC. Program wrap-around services include outreach, care coordination/case management, and direct eligibility assistance for Medi-Cal enrollment. The Clinic embraces care beyond the health system to provide case management to external community supports which can positively impact the patient’s Social Determinants of Health (SDH), diverse nonclinical factors that influence patient’s lives and their health. Recognizing the importance of SDH factors as contributing to wellness, LBCC collects data—including food insecurity, financial resource strain, education, depression, and other data—to inform diagnoses, program adjustments, and collaborations. As a community-centered health care agency, LBCC delivers medical care to patients with private health insurance, government insurance, and the uninsured and embraces its role as a reliable informational resource for all residents. Patients are individuals, families, co-workers, employees, and neighbors. Our commitment is to the community, patient and non-patient alike. The Clinic is a local resource outside the exam room. LBCC hosts an Annual Symposium. The clinic also regularly is featured in the online local newspaper Stu News, promoting outreach, health tips, and neighborly updates. LBCC is a reliable resource for accurate medical information through press releases, social media, and website.

Women's Health

LBCC is a safety net organization serving women in Orange County by providing the full spectrum of care including walk-in urgent care, specialized treatment of chronic conditions, and ongoing preventive well-patient care. For low-income women lacking health insurance and foregoing regular medical visits, LBCC offers the ability for women to take control of their ongoing health care needs. The Clinic’s goal is to provide quality medical care at a low-cost in order to improve access for the most vulnerable women. At LBCC, disadvantaged women have the opportunity to establish a permanent “medical home,” to pursue preventive care and regain control of their health. LBCC’s women’s health programs include: Pap smear monitoring, breast care, family planning, pregnancy testing, screening and treatment of STD’s, prenatal and postpartum care, and linkage to government funded programs. Regular and preventive care also minimizes cost—both to the community and to the patient—by providing early care for diagnoses which can become increasingly costly as conditions worsen due to delayed treatment. Patients of all ages can additionally seek preventive and primary care, encompassing such activities as vaccinations, age-appropriate screenings, and weight and blood pressure monitoring.

Outcomes

By providing low-cost quality medical care by our staff of multi-lingual medical professionals, through our programs, LBCC will achieve the following positive outcomes for its patients:
 
·        Increased access to low-cost, urgent medical care and for some conditions, government-funded care
·        Increased efficiency of medical care delivery as first-time patients in the urgent care program transition into becoming regular patients
·        Reduced community cost and Emergency Room misuse as clients become regular patients and pursue preventive care and annual exams, thereby minimizing emergency room visits
·        Improved health and reduced mortality and morbidity rates, particularly as urgent care patients have the opportunity to establish a “medical home” at LBCC and pursue preventive treatment

HIV/AIDS

A nationally recognized leader in the delivery of individualized, comprehensive medical care and treatment to people living with HIV/AIDS, LBCC is the only resource for low-cost HIV services in South Orange County. LBCC is a Ryan White Care Act subgrantee. LBCC removes barriers to testing and treatment meeting the patient needs: welcoming uninsured patients, accepting those with Medi-Cal, and offering cost-free HIV testing. The availability of anonymous testing at the clinic ensures patient privacy. The culturally sensitive and linguistically capable staff encourages HIV testing for at-risk population groups, including patients of Hispanic ethnicity. HIV tests are administered by registered nurses and available daily. LBCC offers multiple testing options, including standard blood draw, Abbott Architect Ag/Ab, and 20-minute OraQuick tests. LBCC is one of a few clinics in the county offering anonymous testing. LBCC is listed as a registered provider on the national directory of healthcare centers offering PrEP/PEP. Studies have shown that PrEP reduces the risk of getting HIV from sex by 98% when used consistently. This program provides comprehensive care and treatment for persons with HIV/AIDS and includes HIV Boarded primary care physician care, case management counseling, pharmacist counseling, and transmission prevention. As HIV/AIDS patients establish their medical home at LBCC, ongoing program services are extensive and follow a continuum of care ranging from referrals to social service, substance abuse, behavioral health, and drug assistance programs; smoking cessation and nutritional counseling; vaccines; screening for other STDS, tuberculosis, and toxoplasmosis. LBCC exceeds national standards for the quality of HIV/AIDS care delivery.

We have identified the following objectives for this program:
1. Maintain the current quality of care, as measured by the number of patients with CD4 levels indicating functioning immune systems
2. Control spread of HIV, by successful treatment of patients resulting in viral load levels below detectable levels and through counseling
3. Extend lives of patients with HIV, by providing low-cost treatment which enable consistent, regular, accessible physician care through a “medical home”
4. Prevent unnecessary and costly ER visits, by providing an cost-appropriate alternative for non-urgent needs
