Safe Families Program

Safe Families Program was founded in 2016 to address the underserved demographic of women and children who are victims of Domestic Violence/Intimate Partner Violence (DV). The program aims to bridge service gaps by providing trauma-informed victim services that offer wraparound support in their healing journey. We offer crisis prevention services, mental health counseling, case management, and other supportive services that address their immediate needs while providing practical tools that lead to self-sufficiency.

Outcomes

1. 98% of the participants received an Individualized Treatment Recovery Plan (ITRP) to achieve their comprehensive health and stability goals.
2. 98% of participants completed trauma, mental health, and substance abuse disorders assessments and received subsequent community resources, service referrals, or direct care linkages.
3. 75% of participants obtained and remained in permanent housing or exited to permanent destinations.
4. 64% of participants obtained or increased income from non-employment sources and 35% of participants increased income from employment.
5. 72% of participants reported increased life skills, coping skills, and the ability to access services independently by program exit.
6. 65% of participants received counseling from onsite mental health professionals reflecting improved mental health outcomes.

Care Coordination Services

The program provides case management, benefits access, healthcare referrals, CalAIM services, and homeless-prevention services to those chronically homeless, near homelessness, and low-income individuals and families in Orange County

2) Reduce health disparities within low-income individuals, families, and whole communities;
3) Increase housing access to the homeless population, especially those who need medical oversight;
