PROGRAMS
Enhanced Adult Day Care
Available Monday through Friday at our Garden Grove location, this program integrates specialized dementia activities into the Adult Day Care services for individuals diagnosed with mild cognitive impairment, Alzheimer’s disease, or other dementias. It features small group sessions with a staff-to-participant ratio of 1:5, focusing on activities that engage the senses, promote wellness, and enhance memory.
85% of the participants attending on prescribed days will display improvement in mood and self satisfaction and decrease in signs and symptoms of depression as measured by the pre-enrollment and every 6 month administration of the Geriatric Depression Scale.
Adult Day Health Care/Community Based Adult Services
Available Monday through Friday at our Garden Grove location, this program offers personalized care based on individual needs. It includes supervised medical care with a multi-disciplinary assessment, tailored care plans, vital signs and medication monitoring, as well as physical, occupational, or speech therapy. The program also provides personal care services, dietary counseling, and psychological support. Additional services include transportation to and from the center, supervision, a light breakfast, lunch, and a snack, as well as social work services and therapeutic activities.
70% of the participants who attend the recommended number of days and engage in the interventions and activities prescribed in their plan of care will remain in the community, maintain or improve their overall health status and delay institutionalization.
