Alzheimer's Family Center
AFC offers two programs: New Connections, for those with early stage dementia, and Friendship Club, for those with moderate to advanced dementia. All of our programs are evidence-based and are designed to be cognitively stimulating. In addition to brain games, socialization, art and music therapy, and physical therapy, we provide nursing oversight of our patients, snacks and nutritious lunches. Transportation resources are provided. In addition, caregivers receive respite while their loved ones are in our care. We offer free caregiver counseling, which is funded by the UniHealth Foundation.
In our last caregiver survey--conducted in August 2023--over 95% of respondents agreed that having their loved one attend AFC has helped them delay institutionalization. Our last study showed that within twelve months of engaging in ADHC, patients exhibited a 54% reduction in emergency room visits, and a 60% reduction in acute admissions. Patients who attended ADHC experienced a 57% decrease in falls after twelve months of attending.
