PROGRAMS
Older Adults Program
Provides onsite supportive housing services and activities to promote older adult health and independence, encourage an active lifestyle and community support, and reduce isolation. Residents are connected to health and wellness resources and technology access is centered on the needs of older adults. Budget provided is for the Older Adults programs across all CHW communities.
1. 90+ percent of participants will agree that social activities at their community helped them feel less isolated and connected to their neighbors.
2. 80+ percent of participants will agree that the connection to resources helped them access services they needed to maintain their health and wellbeing.
3. 80+% of participants will agree that technology access and support helped them access the services they needed and further reduced isolation.
4. 85% of participants who participate in CHW’s Financial Well-Being Programs will agree these programs helped them remain financially secure.
