Family Access Point Network

The FSC, Family Access Point Network is comprised of 20 agencies working together as a functional system that prevents and ends family homelessness in Orange County in an equitable, coordinated and timely manner. ACCESS POINTS are defined as “entry points” where families can receive services and assistance through the the Orange County Family Coordinated Entry System (FCES) that offer access to Prevention, Diversion, Service Navigation and Assessment.

Outcomes

1. Improve the speed, accuracy and consistency of screening and intake to reduce the average length a family experiences homelessness and ensure that family homelessness is brief and non-recurring.
2. Enhance Orange County's family homeless-serving system's ability to utilize resources efficiently and without duplication of services.
3. Support and enhance Orange County's family homeless response system and advance systems changes.
4. Ensure programs and community response to family homeless needs are best-practice, data-informed and community-needs driven.
5. Improve collaboration and communication between Orange County Systems of Care, Providers, Community and Stakeholders.
6. Shall be committed to Diversity, Cultural Competency, Inclusion and Nondiscrimination in the Provision of Services.

Family Coordinated Entry System

The FSC manages the Family Coordinated Entry System in Orange County, and we developed this system to ensure families are able to access housing support services in an equitable, timely, and coordinated manner, that is designed to match families experiencing homelessness with the appropriate available housing services. This process helps prioritize resources based on vulnerability and severity of service needs to ensure that families who need help the most can receive it. The goal of Family Coordinate Entry System (FCES) is to connect families at‐risk of homelessness or experiencing homelessness to appropriate services, shelter and housing interventions to secure permanent and stable housing in Orange County. For FCES to achieve a sustainable, efficient system, it is necessary that strong partnerships are formed with FCES Access Points, supportive services agencies, and Shelter and Housing Providers. Access Points are agencies that serve as the entry points to CES as well as offer additional assistance via referrals and other services to help support households experiencing a housing crisis. Access Points can range from navigation centers, emergency shelters, food pantries, street outreach programs and more. Supportive Services Agencies provide specialized supportive services to assist with finding and keeping stable, permanent housing. Supportive services include housing navigation, housing stabilization, and housing location services. Shelter and Housing Providers are agencies that provide shelter or housing resources for people at risk of homelessness or experiencing homelessness. Housing resources include shelter, supportive services, homelessness prevention assistance, rapid rehousing, permanent supportive housing and other permanent housing.

Outcomes

There are Five Elements of the Family Coordinated Entry System
Access: The point(s) of entry for services through coordinated entry by providing direct support through training, technical assistance and case conferencing to the FSC Access Points and family shelters.
Assessment: The tool(s) used to determine a household’s eligibility and prioritization for available housing resources.
Diversion/Problem-Solving: The process for resolving an episode of homelessness quickly by exploring a household’s existing supports and offering one-time financial assistance, as needed.
Prioritization: The process for determining the most vulnerable households for immediate housing placement.
Referral/Placement: Once prioritized, households are matched with the best-fitting housing resources.
