Housing Programs

Pathways of Hope operates Via Esperanza, an immediate response shelter for homeless families in Anaheim. Additionally, Pathways operates a Permanent Supportive Housing (PSH) program for chronically homeless families and a PSH program for chronically homeless individuals. We also provide permanent affordable housing.

Outcomes

Pathways of Hope implements sustainable solutions across the housing continuum. We currently offer:

* Emergency Shelter at Via Esperanza for homeless families in Anaheim

* Two Permanent Supportive Housing (PSH) programs with wraparound services for chronically homeless families and individuals

2024 Shelter and Permanent Supportive Housing Programs Outcomes:

* 195 - Total Individuals served through our emergency shelter and PSH programs

* 46 - PSH Participants - Formerly homeless families and individuals permanently housed through our PSH programs

* 71 - Emergency Shelter Participants - Who exited to successful housing

* 68 % - Success Rate - Transitions from emergency shelter to successful housing

Transition to Permanent Housing:

During 2024, our properties on Amerige Avenue in Fullerton, also known as Hope's Corner, transitioned from predominantly shelter operations to permanent affordable housing. By increasing the availability of permanent affordable housing in North Orange County, Pathways of Hope is contributing to the best solution to end homelessness. Thanks largely to a $1.5 million grant from CalOptima, our Amerige Avenue properties have been completely renovated. This transformational financial support gave us the resources to remodel 14 units (two four-bedroom houses, two three-bedroom houses, seven two-bedroom units, two studio units and one ADU unit). Hope's Corner now offers a permanent home to families and individuals as well as housing for transitional-age youth, a service offered by our partners at Orangewood Foundation and Young Lives Redeemed.

Community Based Services

The HUB of Hope is Pathways' main community access point from which the agency offers Community Based Services including Food Pantry Services. Participants of Food Pantry Services receive free, nutritious, culturally relevant groceries once a week--up to 4 times per month--and services are as low barrier as possible. Food Pantry Services regularly serve as a gateway to connect clients to additional Community Based Services like Homelessness Prevention in the form of rental assistance and security deposits, Homelessness Diversion, and connections to our community partners like CalOptima. Pathways is contracted with CalOptima as part of the CalAIM initiative to streamline the clinical and non-clinical needs including housing of its highest-need enrollees. We provide four CalAIM Community Supports: Housing Navigation, Housing Deposits, Housing Tenancy and Sustainability, and Day Habilitation. Additionally, Pathways of Hope operates Food Pantry Services at Cypress College and North Orange Continuing Education (NOCE), available to students enrolled in the North Orange County Community College District (NOCCCD).

Outcomes

Pathways provided the following outcomes through our Community Based Services in 2024:

Pathways of Hope's homelessness prevention services help tackle the ongoing housing crisis by providing financial support to households at risk of eviction. Our goal is to keep people in their homes and support long-term stability. Our dedicated case managers at Pathways of Hope work closely with families and individuals to prevent homelessness with a variety of services. In 2024, Pathways served 233 households with housing navigation services including:

* 125 - Homeless Prevention - Households at risk of eviction remained housed through prevention services, which include rental and utility assistance
* 50 - Shelter Connections - Households connected to shelter services
* 19 - Diversion - Households were diverted from entering the shelter system by identifying alternative housing arrangements like staying with friends or family
