PATH operates Yale Navigation Center, Care Coordination and Encampment Resolution, Rapid Re-Housing, Outreach and CalAIM services.

Outcomes
Over a 12-month period, we will evaluate success through the following:
- Serve 2,094 unhoused individuals across Orange County
- Offer shelter, outreach, housing solutions and case management services to 100% of clients
- Help 314 people move into permanent homes
