SparkPoint OC - An Abrazar Financial Empowerment Program
SparkPoint OC is Abrazar's comprehensive financial empowerment program that helps low-income individuals and families achieve long-term financial stability and economic mobility. Operating at six community-based sites throughout Orange County, the program serves a minimum of 300 families annually through individualized financial coaching, goal setting, financial education, and direct financial assistance. Each participant works one-on-one with a Financial Coach to develop a personalized action plan focused on increasing income, reducing debt, improving credit, building savings, and strengthening overall financial health. The program provides comprehensive support that includes free tax preparation services for more than 1,400 households annually, free Individual Taxpayer Identification Number (ITIN) application processing, public benefits enrollment assistance, workforce and career development, housing stability resources, and financial capability workshops. Participants also benefit from seamless referrals to Abrazar's broad network of internal programs and trusted community partners, ensuring they receive coordinated services that address the many factors affecting financial well-being. By addressing multiple barriers simultaneously, SparkPoint OC helps families build lasting financial resilience and long-term economic mobility.
Participants work with Financial Coaches to establish and achieve measurable short-term and long-term financial goals that strengthen household stability and self-sufficiency. Outcomes include:
(1) Develop and maintain a sustainable household budget that supports long-term financial stability.
(2) Increase household income through employment, career advancement, workforce development, or access to new income opportunities.
(3) Reduce household expenses through improved financial management and resource planning.
(4) Improve credit scores and overall financial health by addressing credit challenges and establishing positive financial habits.
(5) Reduce consumer debt while increasing emergency savings and overall financial resilience.
(6) Enroll eligible individuals and families in public benefits and community resources that improve household stability.
(7) Successfully prepare and file federal and state income tax returns while maximizing available tax credits and refunds through free tax preparation services.
(8) Obtain Individual Taxpayer Identification Numbers (ITINs) for eligible individuals, expanding access to financial services and tax compliance.
(9) Increase housing stability by connecting participants to housing resources, financial assistance, and supportive services.
(10) Achieve individualized short-term and long-term financial goals that lead to greater economic security, self-sufficiency, and overall quality of life.
Home Visitors Program (HVP)
Abrazar's Home Visitors Program (HVP) is an evidence-based early childhood and family strengthening program funded by First 5 Orange County. Through the nationally recognized Healthy Families America (HFA) and Parents as Teachers (PAT) home visiting models, certified Parent Educators partner with expectant parents and families with young children to promote healthy child development, positive parenting practices, school readiness, and long-term family well-being. The program primarily serves families who are participating in or eligible for CalWORKs. Abrazar assists eligible families with the CalWORKs application process to help them access home visiting services and other supportive resources. Referrals are received primarily from the Orange County Social Services Agency (OCSSA), hospitals, prenatal and maternal healthcare providers, pediatric medical providers, and other trusted community partners serving expectant parents and families with young children. Under the supervision of the Home Visiting Program Director, six certified Parent Educators each serve approximately 30 families annually through individualized home visits, developmental screenings, parent education, family goal setting, and connections to healthcare, education, and community resources. Families receive culturally responsive, strengths-based support tailored to their unique needs, helping parents build confidence while fostering safe, nurturing, and developmentally appropriate home environments. To address immediate family needs and reduce barriers to healthy child development, each participating family receives up to $500 in material goods assistance to purchase essential items such as diapers, formula, clothing, car seats, cribs, educational materials, safety equipment, and other necessities that support the health and well-being of both the child and family.
(1) Strengthen positive parenting practices and increase parents' knowledge of child development through evidence-based home visiting services.
(2) Improve children's health, developmental, and school readiness outcomes through regular developmental screenings and individualized family support.
(3) Increase parent confidence, resilience, and protective factors that promote healthy family functioning and positive parent-child relationships.
(4) Connect eligible families to CalWORKs, healthcare, early childhood education, and other supportive services that improve family stability and long-term outcomes.
(5) Promote safe, stable, and nurturing home environments that support healthy child growth and development.
(6) Reduce risk factors associated with child abuse and neglect while strengthening family protective factors.
(7) Assist families in obtaining essential material goods that improve child safety, health, and overall family well-being.
(8) Increase access to healthcare, developmental services, and community resources through coordinated referrals and ongoing case support.
(9) Support families in achieving individualized goals related to parenting, education, employment, family stability, and economic self-sufficiency.
(10) Empower parents with the knowledge, skills, confidence, and resources needed to help their children thrive from pregnancy through early childhood.
CalAIM Community Supports Program
Abrazar's CalAIM Community Supports Program provides comprehensive, person-centered care coordination for eligible CalOptima Health Medi-Cal members with complex medical and social needs. Through an integrated team of Enhanced Care Management (ECM) Care Managers, Community Health Workers (CHWs), and Housing Navigators, the program helps members overcome barriers to healthcare, stable housing, behavioral health services, and other essential community resources. The multidisciplinary team includes five Enhanced Care Management Care Managers, five Housing Navigators, one Community Health Worker, and a Program Director, who work collaboratively to develop individualized care plans that address each member's unique medical, behavioral, and social needs. Services are delivered through a whole-person approach that emphasizes care coordination, health education, advocacy, housing stability, and connections to community resources. Housing services include Housing Transition Navigation, Housing Deposits, Housing Tenancy and Sustaining Services, and assistance connecting members to permanent housing opportunities and supportive services. Enhanced Care Management provides intensive care coordination for individuals with complex health conditions, while Community Health Workers help members navigate healthcare systems, access community resources, and build the skills necessary to improve their overall health and well-being. Although the program currently serves CalOptima Health members, Abrazar continues to expand partnerships that will increase access to these critical services for additional Medi-Cal populations in the future.

(5) Improve members' ability to manage chronic medical conditions through education, coaching, and ongoing care coordination.
(6) Increase successful engagement with healthcare providers, specialists, and community-based organizations.
(7) Reduce avoidable emergency department visits and hospitalizations through proactive care coordination and early intervention.
(8) Improve health equity by providing culturally responsive, trauma-informed services that address the social drivers of health.
(9) Strengthen members' self-advocacy skills, independence, and ability to successfully navigate healthcare and social service systems.
(10) Improve overall quality of life by helping members achieve individualized goals related to health, housing stability, financial security, and long-term well-being.
Orange County Mobility Management & Senior Non-Emergency Medical Transportation (SNEMT) Program
Abrazar's Orange County Mobility Management & Senior Non-Emergency Medical Transportation (SNEMT) Program provides safe, reliable, and accessible transportation that enables older adults and individuals with disabilities to maintain their health, independence, and quality of life. As one of Orange County's largest nonprofit transportation providers, Abrazar operates a fleet of 65 wheelchair-accessible vehicles, delivering more than 120,000 one-way passenger trips annually throughout the county. Transportation services are coordinated through a comprehensive mobility management model that matches riders with the most appropriate transportation option based on their medical, mobility, and personal needs. While the program supports a variety of essential transportation needs, its primary focus is on providing transportation to medical and healthcare appointments, including primary care, specialty care, dialysis, chemotherapy, radiation therapy, rehabilitation, behavioral health services, pharmacies, and other medically necessary destinations. Beyond transportation, Mobility Management helps older adults and individuals with disabilities navigate available transportation resources, identify appropriate mobility options, and connect with community services that support healthy aging and independent living. Through strong partnerships with healthcare providers, senior centers, community organizations, and public agencies, Abrazar helps ensure transportation is not a barrier to receiving essential healthcare and remaining active within the community.

(4) Reduce missed medical appointments by providing dependable, accessible, and customer-focused transportation services.
(5) Promote independence and successful aging by enabling older adults to remain safely in their homes and communities.
(6) Increase mobility options through individualized Mobility Management services that connect riders with the most appropriate transportation resources.
(7) Improve quality of life by reducing social isolation and increasing access to community services, nutrition programs, shopping, and other essential destinations.
(8) Provide safe, ADA-accessible transportation through a professionally trained team of drivers operating a fleet of wheelchair-accessible vehicles.
(9) Strengthen partnerships with healthcare providers, public agencies, and community organizations to improve coordinated transportation services throughout Orange County.
(10) Advance health equity by ensuring vulnerable older adults and individuals with disabilities have reliable transportation to the healthcare and support services they need to remain healthy, independent, and engaged in their communities.
P.R.I.D.E. (Potential, Responsibility, Innovation, Determination, and Excellence) After School Program
Abrazar's P.R.I.D.E. (Potential, Responsibility, Innovation, Determination, and Excellence) After School Program is a free, comprehensive expanded learning program operated in partnership with the Westminster School District through California's After School Education and Safety (ASES) Program, established by Proposition 49. For more than 15 years, Abrazar has partnered with the Westminster School District to provide high-quality after-school programming that helps students succeed academically while developing the character, leadership, and life skills needed for future success. Beginning with the 2026–2027 school year, Abrazar will operate the P.R.I.D.E. Program at six Westminster School District elementary and middle school campuses, serving more than 600 students each school day. Together with the Westminster School District and the Boys & Girls Club of Westminster, the partnership provides expanded learning opportunities that benefit students and families throughout the district. The program inspires students to reach their full Potential, demonstrate Responsibility, embrace Innovation, persevere with Determination, and strive for Excellence in all aspects of their lives. Students participate in homework assistance, academic enrichment, STEM education, literacy, arts and cultural activities, leadership development, physical fitness, nutrition education, and social-emotional learning in a safe, supportive, and inclusive environment. Participation is provided free of charge, ensuring every student has access regardless of family income. In addition to the school-year program, Abrazar partners with the Westminster School District and the Boys & Girls Club of Westminster to operate a free summer expanded learning program, helping students continue learning, build friendships, participate in enriching activities, receive nutritious meals, and return to school ready to succeed.

(7) Provide a safe, supervised, and enriching environment that supports working families during after-school hours and summer months.
(8) Strengthen family engagement through regular communication, family events, and connections to school and community resources.
(9) Remove barriers to participation by providing high-quality after-school and summer programming at no cost to families.
(10) Prepare students for long-term academic success, responsible citizenship, and lifelong learning by helping them develop the skills and character needed to succeed in school and beyond.
