CFCOC - Early Childhood Policy Framework Project

At CFCOC, we believe that every child in Orange County should grow up with access to high-quality care, learning, and opportunity, regardless of their background, abilities, or ZIP code. That belief is the heart of the Early Childhood Policy Framework (Policy Framework), a bold, community-driven effort to ensure all systems serving young children align around equity, prevention, and wellness. Initially developed by community partners and entrusted to CFCOC in 2024, the Policy Framework brings together a growing coalition of parents, early childhood experts, culturally rooted organizations, and disability inclusion advocates to design policies that reflect the real lives and needs of Orange County families. The group currently comprises 25–30 organizations and parent leaders, with plans to expand to 70 partners that reflect the full racial, ethnic, and cultural diversity of Orange County, including Deaf and hard-of-hearing families, Blind and Low-Vision communities, and children and families with disabilities. This initiative is grounded in a Theory of Change that addresses multiple sectors—health, mental health, early learning, and family resilience—through a systems-based approach. Together, the Coalition: • Implementing the Policy Framework, updated to meet today’s needs • Creating a shared, equity-centered work plan • Engaging parent advocates with lived experience • Building relationships across public and private systems • Identifying and preventing policies that harm vulnerable children • Launching a multi-channel outreach and education campaign to reach over 4,000 families Through podcasts, flyers, social media, community speaking, and collaborative events, the goal is to make the Framework not just a document, but a movement. This is about more than fixing services: it’s about reimagining how we care for children and families as a community. By working together rather than in silos, we can reduce disparities, improve environments, and build a more just and resilient Orange County for generations to come.

Outcomes

The Early Childhood Policy Framework Workgroup will continue to implement the revised community-informed Framework, engaging over 70 participants, including culturally rooted CBOs, families with lived experience, and policy leaders. Together, they will create a living work plan for system change, collaborate with public agencies, and elevate family voices in local decision-making. This work will ultimately support tens of thousands of children and families in Orange County by aligning early childhood systems around equity, prevention, and collective impact

Every Family Home Visiting Collaborative - Collaborative Support Organization

While many families in Orange County face barriers to accessing prevention services—such as language barriers, stigma, or a lack of trust in systems—home visiting can be a powerful way to build relationships, promote child development, and strengthen family well-being. The Every Family Home Visiting Collaborative (Every Family HVC) was created by and for the community to ensure that home visiting services are accessible, culturally grounded, and equity-centered. CFCOC proudly serves as a Collaborative Support Organization for this initiative, supporting implementation partners as they deliver trauma-informed, long-term home visiting to families who need it most. Every Family HVC prioritizes: • Families with children ages 0-5 • Pregnant or parenting individuals facing economic hardship, housing instability, or behavioral health challenges • Communities historically excluded from mainstream prevention services, including AANHPI, MENA, Native Indigenous families, and families of children with disabilities Through Every Family HVC, families receive: • In-home parenting and child development support • Navigation of health, insurance, and social services • Emotional wellness and maternal mental health resources • Connections to early learning, nutrition, and wraparound services Every Family HVC is helping ensure that every parent, no matter their background, has the tools, confidence, and support to give their child a strong start in life. This work not only strengthens individual families but also sets a new standard for equity in home visiting: one that is inclusive, community-led, and rooted in trust.

Outcomes

Every Family HVC is preparing to serve 100 additional families by June 2026, with culturally and linguistically responsive home visiting services provided by trusted messengers from the community. Through expanded partnerships, workforce training, and access to wraparound care, families will gain the tools and support needed for long-term health, resilience, and school readiness. The Collaborative is also strengthening data sharing and cross-sector alignment to make home visiting more equitable, effective, and accessible throughout the county.

CFCOC - PATH CITED & Technical Assistance Marketplace

Through California’s CalAIM initiative, a new opportunity emerged for community-based organizations (CBOs) to deliver essential health and social services through Medi-Cal. However, many smaller, culturally rooted nonprofits — those most trusted by immigrant, refugee, and underserved communities — lacked the necessary infrastructure to meet the new system demands. That’s where CFCOC stepped in. With support from PATH CITED Round 2 funding and our role as an approved vendor in the PATH CITED Technical Assistance Marketplace (TAM), we provide hands-on support to help CBOs become Enhanced Care Management (ECM) providers. Over the past 18 months, CFCOC has helped 15 nonprofits across Orange County: • Build the systems needed to deliver ECM services • Train and onboard staff for trauma-informed, culturally responsive care • Implement easy-to-use data and referral platforms • Navigate contracting and engagement with CalOptima and other Managed Care Plans • Develop plans for long-term sustainability once funding is no longer available. Our approach isn’t one-size-fits-all. We meet each organization where they are, co-design practical tools, and foster peer learning through real-time workgroups and collaboration. The result: a stronger, more connected network of frontline providers who can reach families often overlooked by larger systems. When we support these organizations, we amplify the voices of communities who have too often been left behind, and ensure every family has access to the care and coordination they need to thrive.

Outcomes

Through PATH CITED Round 2 and the TA Marketplace, CFCOC has supported 15+ CBOs in building the capacity to become sustainable Medi-Cal providers of Enhanced Care Management and Community Supports. Each organization has completed readiness assessments, implemented care coordination tools, trained staff, and developed sustainability plans. As a result, hundreds of families across Orange County, many of whom previously lacked access, are now being served through trusted, community-based programs equipped to deliver high-quality, culturally responsive care.
