Community Health Access Program

CHIOC’s Community Health Access Program focuses on Outreach, Enrollment, Retention, and Utilization services to connect individuals with the resources they need to lead healthy lives. Our team engages with the community, offering culturally and linguistically tailored information and education on available healthcare and social service benefits and programs. They also provide personalized one-on-one assistance to clients, helping them determine eligibility and enroll in programs such as Medi-Cal, Medicare, Covered California, CalWORKs, General Relief, and CalFresh benefits. Additionally, staff follow up with clients to confirm coverage, offer further education and navigation support, assist in selecting a primary care provider, address any questions, eliminate barriers to care, and help with renewal applications. Furthermore, CHIOC conducts “Know Your Benefits” workshops to help clients comprehend their coverage and develop the skills needed to advocate for themselves in obtaining quality care.

To make Health For All a reality, the Community Health Access Program works to raise awareness of available programs and resources, increase access to healthcare and social services, and improve utilization by connecting clients to culturally competent and bilingual service providers

Regional Health Leadership

Our Regional Health Leadership seeks to reduce fragmentation of healthcare delivery and improve countywide systems through better service coordination. Under this program, CHIOC facilitates the OC Health Access and Enrollment Task Force for training and coordinating enrollment staff and organizations countywide. This task force is comprised of 250+ members from nearly 40 agencies, and our monthly meetings serve as a platform for collaboration and information sharing on local provider services, health plan trainings, policy updates, and networking. We also support other county-level initiatives, such as helping launch the Covered OC collaborative during ACA rollout, working with the County to serve hard-to-reach populations, coordinating with food distribution programs under the CalFresh Collaborative, and sponsoring community forums on health access and immigration. CHIOC also engages in local and statewide advocacy.
