Meals on Wheels

The Meals on Wheels program is for homebound older adults who are unable to shop and/or prepare meals for themselves. Meals on Wheels Orange County delivers breakfast, lunch, and dinner directly to their homes Monday-Friday by friendly volunteer drivers who also provide a regular well-check. Participants also receive case management, which helps participants evaluate and find acceptable solutions to meet their individual needs. Meals on Wheels OC also has a Friendly Visitor Program that connects caring community volunteers with isolated Meals on Wheels participants who benefit from the social interaction. The Home Safety Program provides home safety inspections and repairs in the homes of the frail Meals on Wheels participants. For Meals on Wheels participants who need in-home services, this program arranges for paid caregivers to provide basic personal care and chore assistance for very low income seniors.

Meals on Wheels will help older adults retain their independence and stay healthy by providing not only meals, but also wellness assessments, home safety, personal care resources and referrals to community services.

Adult Day Services

Meals on Wheels Orange County provides two types of Adult Day Services: Adult Day Care (social model) and Adult Day Health Care (the medical model). Our Adult Day Care Center is located in Buena Park at the Buena Park Senior Center and offers support and daytime respite to families and friends who are caring for a frail older loved one with memory impairment, dementia, and early stages of Alzheimer's disease. The program provides a safe and comfortable day time environment where seniors can enjoy games, crafts and social interaction with their peers. Meals on Wheels' Adult Day Health Care Centers are located in Anaheim offer comprehensive day-time care and licensed medical supervision to functionally impaired adults. This program often helps participants avoid placement in nursing homes or other full-time care facilities while providing caregivers day-time support and rest from caregiver responsibilities. Services offered include door-to-door transportation, medication management, meals, activities, and physical and occupational therapy.

Adult Day Services will help keep families together by offering a unique set of services to meet needs in a friendly neighborhood setting. Clients will be part of a warm environment wtih social activities, and compassionate care for those with a disability or who are aging, as well as personal attention to daily medical routines. A choice of activities, exercises and social interaction helps maintain physical and mental health and makes it possible to remain living at home. This provides peace of mind to family members and caretakers, assuring them that their family member is safe, socially energized and receiving needed nutrition and medical attention during the day.

Care Coordination

Building on our partnerships with area hospitals to improve care coordination and reduce hospital readmissions for older adults, Meals on Wheels Orange County has formed an innovative collaborative with a senior housing complex to provide case management services to residents who were homeless or at-risk of homelessness. These residents typically have 6 to 7 separate diagnoses, e.g., diabetes, mental health issues, congestive heart failure. Our program provides these residents with Meals on Wheels and daily visits by our care managers who assist with chronic disease management, medical appointments, access to health care, access to mental health group therapy, and legal assistance. Thanks to the success of this program, we are looking to partner with other low-income senior housing programs to deliver these life-changing services to their residents. In addition, Meals on Wheels OC also provides these services under contract with major healthcare plans to support person-centered solutions.

Outcomes

Hospital re-admissions for Medicare patients will be reduced due to the Care Transitions program administered by Meals on Wheels Orange County. The program will help the participants get to follow-up medical appointments, take their prescribed medications and have access to healthy meals and transportation.

This will save Medicare and our health care systems hundred of thousands of dollars.

Lunch Café

Hot, nutritious lunches are provided at 24 senior and community centers in central and north Orange County to people 60 years and older. Besides maintaining the physical health of older adults, these programs also work to enrich the emotional/psychological health of older adults by sponsoring a wide range of activities.

The Meals on Wheels Orange County nutrition program not only enhances the physical and mental well-being of our older population but serves essential human needs by providing a space for active older adults to eat out, socialize around a nutritious meal served by volunteers and staff, meet people with shared interests and join in other activities the center and the community offers.
